We are a new clinic and are currently in the process of being credentialed with multiple vision plans and medical insurances. At this time, we are only offering self-pay exams. We provide you with the necessary documents to self-submit to your insurance provider for reimbursement.

Optical Department

Vista Eyecare & Wellness is independent from Costco Optical. They offer a full-service optical department with a variety of eyewear and designer frames for everyone. They offer expert guidance to help you select the perfect pair of glasses. They can assist you with all of your glasses and contact lens ordering, as well as insurance verification for benefits of materials. You need to be a Costco Member to use Costco Optical services.

Clinic Policies

By scheduling an appointment at Vista Eye Care, you agree to the terms outlined below;

1. Billing and Insurance information MUST be provided prior to your appointment day. If insurance information is not provided, please expect to pay out-of-pocket and self-submit your claim.

2. We are independent from Costco. We take different insurance plans than Costco Optical. The list of preferred providers on your insurance website may be only one of us or both. Send us your insurance information in advance to confirm and verify in-network benefits for your exam.

3. Retinal Imagery is required and is not covered by insurance. The fee is $25 for ages 18 and over. There is no fee for minors.

4. If you are over 10 minutes late for your appointment, we may have to reschedule.

5. All no-shows and cancellations with less than 24 hours' notice are subject to a $15 fee due at your next appointment.
